Seeking a motivated and compassionate Nurse Practitioner to provide home health risk assessments for Medicare Advantage plan members throughout Kent County, MI. This per diem opportunity allows you to design a schedule that works best for your lifestyle, as shifts are available seven days a week. During visits, you will travel to members homes in Grand Rapids, Wyoming, Kentwood, and other areas of Kent County, performing comprehensive health assessments and supporting members long-term wellness needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule and adjust as needed
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct thorough home health risk assessments in members residences
• Carefully review and document current and past medical history, ongoing conditions, and medication usage
• Educate members on disease management and medication adherence
• Identify care gaps and report findings or recommendations to support member health, without creating treatment plans or prescribing
• Each evaluation typically lasts about 45 minutes
• Travel to various locations within Kent County for scheduled visits

Qualifications:

• Active Nurse Practitioner license in Michigan
• Current Basic Life Support (BLS/CPR) certification
• Board certification through AANP or ANCC
• 6 months of nurse practitioner experience preferred
• MSN from an accredited school of nursing
