Job Description
As a Nurse Practitioner specializing in home health, you'll deliver personalized care by conducting risk assessments for Medicare Advantage members in Kent County, MI. This per diem opportunity offers maximum flexibilityset your own schedule and work in diverse locations such as Grand Rapids, Wyoming, and Kentwood. Each assessment averages 45 minutes and may include annual health evaluations or post-discharge visits.
Schedule:
- Flexible shifts available seven days a week
- Design your own work hours
- Minimum weekly or bi-weekly commitment
Essential Responsibilities:
- Complete comprehensive annual home health risk assessments
- Review and evaluate patients prior/current medical history and medication use
- Deliver education to members regarding their medical conditions and prescribed medications
- Identify care gaps and offer actionable recommendations for follow-up
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Michigan
- Current CPR (BLS) certification
- Board certified through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of six months recent experience as a Nurse Practitioner preferred
Qualified candidates will receive compensation of $80$90 per evaluation, plus a mileage stipend and pay for administrative time.
