Job Description

Deliver comprehensive patient care and make a positive impact in the community by providing in-home health assessments as a Nurse Practitioner on a per diem basis. This role focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Kent County, including Grand Rapids, Wyoming, and Kentwood.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to suit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan participants in their homes
  • Review and evaluate patients medical history, current health conditions, and medication regimens
  • Educate patients on their medical diagnoses and prescriptions
  • Identify and document gaps in care, providing straightforward recommendations to address them
  • No treatment plan creation or prescription-writing necessary

Qualifications:

  • Active Nurse Practitioner license in the state of Michigan
  • Current CPR certification (BLS)
  • Board certification by AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent experience as a Nurse Practitioner preferred
