Schedule:

• Flexible shifts available any day between Monday and Sunday
• Self-scheduling: create a workweek that fits your needs
• Minimum weekly or bi-weekly hourly commitment as per your preference

Key Responsibilities:

• Conduct comprehensive home health risk assessments in patient residences across Cumberland County
• Review and document patients medical histories, known conditions, and medication regimes
• Educate patients regarding their health conditions and treatments
• Identify gaps in care and share actionable recommendations
• Visits may include annual health and wellness assessments or post-discharge follow-up
• No requirement to prescribe medications or develop treatment plans

Qualifications:

• Nurse Practitioner License valid in Tennessee
• Board Certification through AANP or ANCC
• Current CPR certification (BLS required)
• MSN degree from an accredited nursing program
• At least six months of recent Nurse Practitioner experience highly preferred

As part of this role, compensation is structured at $80$90 per evaluation, with an added mileage stipend and compensation for administrative time.
