Job Description

Deliver high-quality patient care as a Nurse Practitioner in a per diem, home health setting. This opportunity allows you to utilize your clinical expertise to make an impact by conducting home-based risk assessments for Medicare Advantage plan members in Cumberland County, TN, including towns like Crossville, Pleasant Hill, and Crab Orchard. Enjoy the flexibility to shape your workweek with shifts of your choosing, Monday through Sunday, and a minimum weekly or bi-weekly commitment tailored to fit your schedule.

Schedule:

  • Flexible shift options available 7 days a week
  • Build your own schedule according to your availability
  • Commit to a minimum number of hours on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review patients medical history, existing conditions, and medication usage
  • Educate patients about their medical conditions and prescriptions
  • Pinpoint gaps in care and offer actionable recommendations
  • Home visits may include annual check-ups or post-discharge evaluations
  • No requirement to create treatment plans or prescribe medications

Desired Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board Certification (AANP or ANCC)
  • BLS/CPR certification
  • MSN degree from an accredited nursing program
  • Minimum of 6 months NP experience (preferred)

  • Compensation of $80$90 per completed evaluation, plus mileage stipend and paid administrative time
