We are seeking an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage members throughout Calhoun County, FL, including Blountstown, Altha, and neighboring areas. This per diem role offers autonomy, flexibility, and the opportunity to tailor your schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own working days and hours
• Satisfy a minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform comprehensive home health risk assessments, averaging 45 minutes each, for Medicare Advantage members in their residences
• Review and evaluate prior and current medical and medication histories, as well as ongoing medical conditions
• Provide education to patients regarding their health conditions, medications, and preventive care
• Identify gaps in care and develop actionable recommendations to close those gaps
• No treatment planning or medication prescribing is necessary for this role

Qualifications:

• Active Nurse Practitioner license in the state of Florida
• Board Certification as an NP through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• Current CPR certification (BLS)
• At least 6 months of recent NP experience preferred
