Advance your career and make a meaningful impact by providing in-home health risk assessments as a Nurse Practitioner specializing in home health. This per diem opportunity allows you the flexibility to design your own schedule while serving Medicare Advantage members throughout Calhoun County.
Schedule:
- Flexible shifts available Monday through Sunday
- Arrange your shifts according to your own availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan participants in various locations within Calhoun County
- Review and assess patients prior and current medical histories, chronic conditions, and medication regimens
- Deliver clear patient education related to their health conditions and prescribed medications
- Identify gaps in care and utilize clinical judgment to make appropriate recommendations aimed at improving patient outcomes
- No requirement to create ongoing treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Current, valid CPR certification (BLS)
- Board Certification through either AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
