We are seeking experienced Nurse Practitioners to conduct home health risk assessments for Medicare Advantage plan members throughout Kent County, MI, including Grand Rapids, Wyoming, and Kentwood. Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
• Assess patients' prior and current medical history, medical conditions, and medication history during in-home visits
• Provide clear, compassionate education on medical conditions and medications to patients
• Identify potential gaps in care and make recommendations for improvement
• Focus strictly on assessment and educationno treatment plan creation or prescription writing required

Qualifications:

• Active Nurse Practitioner license in Michigan
• Current BLS CPR certification
• Board Certification through AANP or ANCC
• MSN from an accredited nursing program
• At least 6 months of current NP experience preferred
