Job Description

Deliver comprehensive care in a flexible, rewarding setting, supporting individuals within their homes as a Nurse Practitioner. This per diem opportunity involves conducting home health risk assessments for Medicare Advantage plan members throughout Grand Rapids and the greater Kent County area, including Wyoming and Kentwood. Create your own schedule, working in various local communities and making a direct impact on patients well-being.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose from minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Perform annual health risk assessments for Medicare Advantage participants in their homes
  • Review patients prior and current medical history, health conditions, and medication records
  • Offer education to patients regarding medical conditions and medication management
  • Assess and identify care gaps, providing practical recommendations for improvement
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in the state of Michigan
  • Masters degree in Nursing (MSN) from an accredited institution
  • Board certification through AANP or ANCC
  • Current Basic Life Support (BLS/CPR) certification
  • At least six months of recent Nurse Practitioner experience preferred

Compensation:

  • $80-$90 per completed evaluation
  • Mileage stipend
  • Administrative time compensated
