Advance your career with a per diem opportunity designed for experienced Nurse Practitioners specializing in home health. This role centers on conducting risk assessments for Medicare Advantage plan members throughout Grand Junction and the surrounding communities within Mesa County, including Fruita and Palisade. The average assessment takes approximately 45 minutes, with a focus on annual visits or post-discharge evaluations. With the flexibility to create your own schedule, you can balance your professional commitments with personal priorities.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Set your own availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members in their residences
  • Review and evaluate prior and current medical history, conditions, and medication use
  • Educate members about their medical conditions and prescribed medications
  • Identify gaps in care and make recommendations for follow-up or resolution
  • No responsibility for treatment planning or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Master of Science in Nursing from an accredited program
  • At least six months experience as a practicing Nurse Practitioner preferred
