Job Description

Experienced Nurse Practitioners are sought for dynamic per diem opportunities conducting home health risk assessments for Medicare Advantage plan members across Muskegon County. This role offers flexibility and autonomy, making it an ideal fit for practitioners looking to custom-tailor their work schedule while making a tangible impact on patient care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments, each approximately 45 minutes, including annual and post-discharge visits for Medicare Advantage members
  • Travel to patient residences across multiple areas within Muskegon County (including Muskegon, Norton Shores, and Muskegon Heights)
  • Review and evaluate prior/current medical history, health conditions, and medication usage
  • Provide patients with education regarding their conditions and prescribed medications
  • Identify gaps in care and offer clear recommendations to close those gaps
  • No medication prescribing or creation of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Board certification (AANP or ANCC)
  • MSN from an accredited nursing program
  • Current BLS/CPR certification
  • Minimum six months of NP clinical experience preferred
  • Professional communication, strong organizational skills, and a patient-focused approach to care

Compensation for this position includes $80$90 per evaluation, with an added mileage stipend and compensation for required administrative time.
