Job Description
Seeking a dedicated Nurse Practitioner with home health experience to conduct risk assessments for Medicare Advantage plan members throughout Genesee County. This per diem opportunity is ideal for providers seeking flexibility, autonomy, and the chance to positively impact patients health outcomes in a community setting.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit personal commitments
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments, averaging 45 minutes per visit
- Evaluate members prior/current medical history, health conditions, and medication regimens
- Educate patients on medical conditions and medication adherence
- Identify care gaps and recommend appropriate follow-up steps
- Home visits throughout Flint, Grand Blanc, Burton, and other Genesee County locations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Michigan
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of six months recent NP experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
- Strong assessment, communication, and patient education skills
