Contribute to impactful patient care as a Nurse Practitioner specializing in home health risk assessments throughout Genesee County, including communities such as Flint, Grand Blanc, and Burton. This per diem opportunity is ideal for practitioners seeking flexibility and the chance to make a tangible difference in the lives of Medicare Advantage plan members.

Schedule:

• Flexible shifts available seven days a weekMonday through Sunday
• Create your own schedule to fit personal and professional commitments
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Complete comprehensive annual home health risk assessments in patients homes (approximately 45 minutes per visit)
• Review and evaluate prior and current medical conditions, medication history, and overall health status
• Deliver patient education regarding disease management, medications, and healthy living
• Identify and document care gaps, recommending solutions to support improved health outcomes
• No prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license issued in Michigan
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing program
• At least six months of experience as a Nurse Practitioner preferred
