Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hour commitment
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members across Genesee County, including Flint, Grand Blanc, and Burton
- Assess patients prior and current medical history, ongoing health conditions, and medication regimens
- Educate patients about their conditions and prescribed medications
- Identify care gaps and make recommendations to improve patient outcomes
- No requirement for treatment plan development or medication prescribing
Qualifications:
- Licensed Nurse Practitioner in Michigan
- Board certified by AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited school
- Current BLS certification (CPR)
- At least 6 months of recent nurse practitioner experience preferred
