Schedule:

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members across Genesee County, including Flint, Grand Blanc, and Burton
  • Assess patients prior and current medical history, ongoing health conditions, and medication regimens
  • Educate patients about their conditions and prescribed medications
  • Identify care gaps and make recommendations to improve patient outcomes
  • No requirement for treatment plan development or medication prescribing

Qualifications:

  • Licensed Nurse Practitioner in Michigan
  • Board certified by AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited school
  • Current BLS certification (CPR)
  • At least 6 months of recent nurse practitioner experience preferred
