Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health assessments. This position focuses on conducting thorough risk evaluations for Medicare Advantage plan members living in Marion County, WV, including communities such as Fairmont, Mannington, and Pleasant Valley. Each home health risk assessment averages 45 minutes and may be an annual review or a post-discharge visit.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and document participants medical history, existing conditions, and current medication usage
• Educate patients on best practices regarding their medical conditions and medications
• Detect gaps in care, advising on potential resolutions to enhance patient outcomes
• Focus solely on assessment; no prescribing or development of treatment plans required

Qualifications:

• Valid and active Nurse Practitioner license in West Virginia
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited school of nursing
• At least 6 months of recent experience as a Nurse Practitioner (preferred)

In this role, youll earn $80-$90 per completed evaluation and receive a mileage stipend and compensation for administrative time, recognizing the importance of your contribution during and after patient visits.
