Job Description

Step into a dynamic per diem opportunity designed for Nurse Practitioners who are passionate about patient-centered care and flexibility. In this role, you'll conduct home health risk assessments for Medicare Advantage plan members throughout Jackson County, FL, including locations such as Marianna, Graceville, and Sneads. Each assessment typically lasts about 45 minutes, ranging from annual evaluations to post-discharge visits, offering the chance to make a significant impact on patient well-being.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Commit to a minimum weekly or bi-weekly hourly requirement

Role Responsibilities:

  • Perform comprehensive annual risk assessments in the patients home environment
  • Review and document prior and current medical history, existing health conditions, and medication usage
  • Educate patients about their health conditions and medications to promote understanding and self-management
  • Detect gaps in care and provide actionable recommendations for improved health outcomes
  • No requirements for treatment plan creation or medication prescription

Preferred Qualifications:

  • Active and unrestricted Nurse Practitioner license in the State of Florida
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • At least 6 months of recent Nurse Practitioner experience is highly preferred
  • Master of Science in Nursing (MSN) from an accredited institution
