Job Description

Make a meaningful impact on patient health by performing in-home risk assessments for Medicare Advantage plan members across Naples, Immokalee, Marco Island, and surrounding communities in Collier County, FL. This per diem opportunity is ideal for Nurse Practitioners who excel in independent work and value flexibility. Visits vary between annual wellness reviews and post-discharge check-ins, each typically lasting 45 minutes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitments as required

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Thoroughly review and evaluate prior/current medical history, existing conditions, and medication use
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care, offering clear recommendations for follow-up or improvement
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active and unencumbered Nurse Practitioner license in Florida
  • Board Certification as an NP via AANP or ANCC
  • Current Basic Life Support (CPR/BLS) certification
  • MSN from an accredited nursing program
  • At least 6 months of recent NP experience is preferred

This independent, field-based role compensates $80$90 per completed evaluation, with additional mileage stipend and payment for administrative time.
