Step into a rewarding per diem opportunity providing vital home health services to Medicare Advantage members throughout Collier County, FL, including Naples, Immokalee, and Marco Island. As an experienced Nurse Practitioner specializing in home health, you will play an important role in improving patient care and supporting overall wellness by performing in-home risk assessments.
Schedule:
- Flexible shifts available seven days a week
- Design your own work schedule
- Opportunities for both weekly and bi-weekly hourly commitments
Key Responsibilities:
- Complete comprehensive annual risk assessments for Medicare Advantage health plan members in their homes
- Evaluate prior and current medical history, medical conditions, and medication adherence
- Educate patients on their medical conditions and prescribed medications during each visit
- Recognize and address gaps in care, providing recommendations to promote positive health outcomes
- Focus solely on assessmentno prescribing or treatment plan development is required
Qualifications:
- Active and unrestricted Nurse Practitioner license in Florida
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of six months recent NP experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers competitive compensation at $80-$90 per evaluation, as well as a mileage stipend and additional pay for administrative time.
