Job Description

This per diem opportunity seeks an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members throughout Jerome County. The work is tailored for professionals who value autonomy and a varied workday, with visits ranging from annual health assessments to post-discharge check-ins. Each evaluation typically takes about 45 minutes, with shifts available throughout areas such as Jerome, Hazelton, and Eden.

Schedule:

  • Flexible shifts available seven days a week
  • Option to create your own schedule
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform comprehensive home health risk assessments, primarily with Medicare Advantage participants
  • Assess and document medical histories, current conditions, and medication information
  • Educate patients regarding their diagnoses, treatment plans, and medications
  • Identify gaps in care, offering actionable recommendations
  • Please note: No treatment planning or medication prescribing is expected

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board Certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • Minimum of 6 months recent experience as a Nurse Practitioner preferred
  • Masters degree in Nursing (MSN) from an accredited institution
