Deliver advanced care and vital support as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, you will perform comprehensive evaluations for Medicare Advantage plan members, assisting patients across West Odessa, Goldsmith, Odessa, and the broader Ector County, TX area. Your visits will focus on annual health assessments or post-discharge reviews, lasting approximately 45 minutes each.

Schedule:

- Create your own schedule with minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and assess current and prior medical history, as well as ongoing conditions and medication use
- Deliver patient education regarding ongoing health concerns and medication management
- Identify gaps in care and recommend follow-up actions
- No requirements for prescribing medication or creating ongoing treatment plans
Qualifications:
- Active and unrestricted Nurse Practitioner license in Texas
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months current NP experience preferred
- MSN from an accredited nursing school
Compensation for this opportunity is $80$90 per completed evaluation, with additional mileage stipend and administrative time included.
