Job Description

Deliver meaningful care directly to patients in their homes as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers you the autonomy to shape your schedule, selecting flexible shifts within Fulton Countyincluding Gloversville, Johnstown, and Broadalbinbetween Monday and Sunday, with the ability to create your own shifts and set a preferred hourly commitment on a weekly or bi-weekly basis.

Role Responsibilities:

  • Conduct annual and post-discharge home health risk assessments, each approximately 45 minutes, for Medicare Advantage members.
  • Review prior and current medical histories, evaluate ongoing conditions, and assess medication use.
  • Educate patients about their health conditions and current medications, supporting ongoing understanding and well-being.
  • Detect gaps in care, providing professional recommendations to help close those gaps.
  • Focus exclusively on assessmentsno prescribing or treatment planning required.

Qualifications:

  • Current Nurse Practitioner license, valid in NY
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • A minimum of 6 months of recent Nurse Practitioner experience preferred
