Schedule:
- Flexible shifts available Monday through Sunday
- Self-directed schedulingchoose your days and hours
- Minimum hourly commitment weekly or bi-weekly
Key Responsibilities:
- Conduct thorough, approximately 45-minute home health risk assessments for Medicare Advantage members
- Evaluate patient histories, existing medical conditions, and medication use
- Deliver proactive health and medication education tailored to each individual
- Pinpoint care gaps and recommend appropriate actions
- No requirements for creating treatment plans or prescribing medications
Qualifications:
- Nurse Practitioner License in New Mexico
- Board Certified by AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited nursing program
- At least 6 months of recent NP practice (preferred)
This role offers compensation of $80-$90 per completed evaluation, including a mileage stipend and a paid administrative time component.
