Schedule:

• Flexible shifts available Monday through Sunday
• Design your own weekly or bi-weekly schedule based on your availability
• Minimum time commitment required per week or bi-weekly

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan participants
• Assess and document medical history, current conditions, and medication usage during home visits (average 45 minutes per visit)
• Educate patients on their medical conditions and prescribed medications
• Identify gaps in patient care and make recommendations for improvement
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in New York State
• Board certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred
