Job Description

Deliver essential healthcare services directly in patients homes as a Nurse Practitioner providing risk assessments on a per diem basis. This role focuses on supporting Medicare Advantage members throughout Desha County, including areas such as Dumas, McGehee, and Arkansas City. Youll play a vital part in helping members understand and manage their health by conducting thorough assessments and offering guidancewithout the responsibilities of prescribing or ongoing care management.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete comprehensive annual risk assessments for Medicare Advantage members in the home setting
  • Review and evaluate patients current and past medical history, ongoing conditions, and medication regimens
  • Educate members on health conditions, medications, and overall health management
  • Identify care gaps and provide practical recommendations to help close those gaps
  • Visits typically last around 45 minutes, varying from annual assessments to post-discharge evaluations
  • Focus on assessment and educationno prescribing or treatment planning required

Qualifications:

  • Unrestricted Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board Certification from AANP or ANCC required
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience is preferred
