Job Description

Enjoy the flexibility of per diem work while making a meaningful impact on patient care in the community. As a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members, youll provide vital evaluations for individuals and families in the wider Shelby County area, including Memphis, Bartlett, and Collierville. Each assessment typically lasts about 45 minutes and may include annual health evaluations or post-discharge visits. Your visits support members well-being and contribute valuable insights into their healthcare needsall without the responsibility of prescribing medications or developing treatment plans.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Create your own schedule and select shifts that suit your lifestyle
  • Minimum weekly or bi-weekly hourly commitments available

Key Responsibilities:

  • Conduct thorough home-based risk assessments for Medicare Advantage members
  • Review medical history, existing conditions, and current medication regimens
  • Educate patients about their health conditions and medications
  • Identify care gaps and recommend actionable solutions
  • Focus solely on assessmentsno medication management or treatment planning

Qualifications:

  • Active Nurse Practitioner license in Tennessee (TN)
  • Current CPR certification (BLS)
  • Board certified through AANP or ANCC
  • Prefer at least six months of current experience as a Nurse Practitioner
  • Masters degree in Nursing (MSN) from an accredited program
