Step into a vital role focused on delivering quality care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity allows you to work throughout Shelby County, conducting evaluations for Medicare Advantage members residing in Memphis and nearby communities such as Bartlett and Collierville. The position offers schedule flexibility and an excellent compensation structure: each thorough assessment, lasting approximately 45 minutes, is compensated between $80 and $90 per visit, with additional mileage and administrative time covered.

Schedule:

• Flexible shifts available seven days a week

Key Responsibilities:

• Complete comprehensive annual risk assessments for Medicare Advantage plan members in their homes
• Review patients prior and current medical history, with detailed evaluation of chronic and acute conditions
• Assess medication usage, educate members on their health and prescribed therapies
• Identify gaps in care and recommend possible next steps
• Facilitate assessments onlyno requirement to prescribe or create ongoing treatment plans

Qualifications:

• Current Nurse Practitioner License in Tennessee
• Valid CPR certification (BLS)
• National board certification (AANP or ANCC)
• MSN degree from an accredited nursing school
• At least 6 months of recent experience as an NP preferred
