Job Description

Make a meaningful impact on patient care in your community by performing home health risk assessments as a Nurse Practitioner in a flexible per diem role. Using your advanced clinical skills, you will visit Medicare Advantage plan members throughout Preble County, providing valuable assessments and education to support their long-term health.

Schedule:

  • Flexible shifts available any day, Monday through Sunday

  • Commitment options to meet your weekly or bi-weekly preference

Responsibilities:

  • Complete annual and post-discharge home health risk assessments, averaging 45 minutes per visit
  • Review, evaluate, and document detailed medical, surgical, and medication histories
  • Provide individualized patient education regarding chronic conditions and medication management
  • Identify and communicate care gaps, offering recommendations to address issues identified during assessments
  • Maintain clear documentationno prescriptive authority or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification (AANP or ANCC)
  • MSN degree from an accredited program
  • Current CPR/BLS certification
  • At least 6 months of recent Nurse Practitioner experience preferred

You will serve diverse areas including Eaton, Camden, Lewisburg, West Alexandria, and nearby communities within Preble County. Each evaluation is compensated at $80-$90, with mileage reimbursement and paid administrative time, ensuring your efforts and travel are valued.
