Deliver expert care and make an impact by providing in-home health risk assessments for Medicare Advantage members. This per diem opportunity is ideal for Nurse Practitioners seeking a flexible schedule while serving communities throughout Williamson County. The role focuses on providing comprehensive annual health assessments and post-discharge visits, working with patients in Mount Pleasant, Winfield, Talco, and surrounding areas.

Schedule:

• Flexible shifts available Monday through Sunday
• Enjoy the freedom to design your own schedule
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Complete in-home annual risk assessments for Medicare Advantage plan members
• Review and document prior/current medical history, conditions, and medications
• Educate patients on medical issues and prescribed medications to foster understanding and compliance
• Identify gaps in care and recommend resolutions to enhance patient outcomes
• Focus solely on assessments (no treatment planning or prescribing required)

Qualifications:

• Active Nurse Practitioner license in Texas
• Current CPR certification (BLS required)
• Board Certified through AANP or ANCC
• MSN from an accredited nursing program
• At least 6 months of recent NP clinical experience preferred
