Take on a meaningful opportunity to provide personalized care as a Nurse Practitioner specializing in home health risk assessments on a per diem basis. In this role, you will visit Medicare Advantage plan members in Nez Perce County, ID, including areas such as Lewiston, Lapwai, and Peck. Each assessment lasts approximately 45 minutes and may include annual health evaluations or post-discharge visits, all focused on improving health outcomes for members in their homes.
Schedule:
- Flexible scheduling options available seven days a week
- Self-scheduling to build shifts that best fit your availability
- Choose your preferred commitment level, with minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication profiles
- Educate patients about their diagnoses and the use of prescribed medications
- Identify gaps in care and recommend steps to address them
- No prescribing or direct treatment plans required
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR/BLS certification
- At least six months of recent Nurse Practitioner experience preferred
For your expertise, youll receive between $80-$90 per evaluation, as well as a mileage stipend and compensation for administrative time.
