Make a direct impact in patient care as an experienced Nurse Practitioner providing home health risk assessments on a per diem basis. This role centers on conducting evaluations with Medicare Advantage plan members in Custer County, OK, including locations such as Arapahoe, Clinton, Weatherford, and Thomas. Each assessment averages 45 minutes and may be scheduled to address annual health requirements or post-discharge needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual risk assessments in patient homes
• Gather and evaluate current/past medical history, health conditions, and medications
• Educate patients about health conditions and medication regimens
• Identify care gaps, offering recommendations to address them
• No need to create treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in Oklahoma
• Active CPR certification (BLS required)
• Board Certification via AANP or ANCC
• Minimum 6 months of experience as an NP is preferred
• Master of Science in Nursing (MSN) from an accredited institution
