Job Description
Deliver impactful patient care in a dynamic home health setting by performing in-depth risk assessments for Medicare Advantage plan members across Hertford County, NC. In this per diem role, youll leverage your clinical expertise as a Nurse Practitioner to visit patients in their homesincluding those in Ahoskie, Murfreesboro, and Wintonensuring both flexibility and rewarding professional experiences.
Schedule:
- Flexible shifts available Monday through Sunday
- Opportunity to create your own schedule
- Set your preferred weekly or bi-weekly commitment
Key Responsibilities:
- Conduct 45-minute home health risk assessments for Medicare Advantage plan members
- Examine prior and current medical histories, evaluate medical conditions, and review medication regimens
- Educate patients regarding their health status and prescribed medications
- Detect care gaps and make actionable recommendations to close them
- No responsibility for initiating treatment plans or prescribing medications
Qualifications:
- Possess a current Nurse Practitioner license (NC)
- Hold an active BLS/CPR certification
- Certified by AANP or ANCC
- Graduate of an accredited MSN program
- Minimum of six months recent Nurse Practitioner experience preferred
