Job Description

Deliver impactful care with flexibility as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, you will conduct comprehensive evaluations for Medicare Advantage members living across Hertford Countyserving individuals in Ahoskie, Murfreesboro, Winton, and neighboring communities.

Enjoy the freedom to set your own schedule, with flexible shifts available between Monday and Sunday and a minimum weekly or bi-weekly hourly commitment. Each home visit averages 45 minutes, and may include annual health assessments or post-discharge evaluations supporting quality, coordinated care.

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Review and document comprehensive medical, medication, and health histories
  • Educate members regarding diagnoses, medications, and health maintenance strategies
  • Identify care gaps and make care recommendations to support improved health outcomes
  • No prescribing or development of treatment plans is required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • MSN from an accredited nursing program
  • Current BLS CPR certification
  • At least 6 months of recent NP experience is preferred
