Job Description

Make a meaningful impact as a Nurse Practitioner by providing comprehensive home health risk assessments on a per diem basis. This role empowers practitioners to serve Medicare Advantage plan members across multiple communities in Lincoln County, including Lincolnton, Iron Station, Lowesville, and Westport. Candidates exercise significant autonomy by creating their own working schedules, supporting improved patient outcomes with thoughtful care visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to best suit personal and professional needs
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual risk assessments and post-discharge visits in patients homes
  • Review and evaluate prior and current medical history, existing conditions, and overall medication use
  • Offer education to patients regarding their health conditions and prescribed medications
  • Identify care gaps and recommend actions to close those gaps, enhancing patient outcomes
  • No prescribing or development of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • MSN degree from an accredited school of nursing
  • Minimum six months of recent Nurse Practitioner experience preferred
