As a Nurse Practitioner conducting home health risk assessments, you will play a vital role in enhancing patient care for Medicare Advantage plan members residing in Gaston, NC, and across Northampton County, including communities such as Garysburg and Rich Square. With a focus on flexibility, this per diem role allows you to manage your own availability and make a direct impact on members' health and well-being in their home environments.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum hourly commitment required on a weekly or bi-weekly basis

Key Responsibilities:

• Conduct thorough annual health risk assessments and occasional post-discharge visits in patients homes
• Review and evaluate medical histories, current conditions, and medication regimens
• Educate members about their health conditions and medications to encourage optimal self-management
• Identify care gaps and recommend appropriate next stepsno treatment plan or medication prescribing required
• Documentation for each assessment (approximately 45 minutes per visit), with mileage and administrative time factored in

Qualifications:

• Licensed Nurse Practitioner in North Carolina
• Board certified by the AANP or ANCC
• MSN degree from an accredited nursing program
• Current BLS (CPR) certification
• At least 6 months of NP experience (preferred, but not required)
