Job Description
Make a meaningful impact as an experienced Nurse Practitioner delivering high-quality home health risk assessments to Medicare Advantage members throughout Grant County. This per diem opportunity allows you to use your clinical expertise to support patient health in a flexible, independent environment.
Schedule:
- Flexible shifts available Monday through Sunday
- Enjoy the autonomy of creating your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Perform annual health risk assessments for Medicare Advantage plan members in their homes
- Evaluate medical history, current conditions, and medication usage during every visit
- Educate patients on health conditions and proper medication adherence
- Identify gaps in care and provide recommendations, supporting patients ongoing wellness
- Visits do not include diagnosis, prescribing, or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board certification (AANP or ANCC)
- Current CPR certification (BLS)
- MSN from an accredited nursing school
- At least 6 months of recent nurse practitioner experience preferred
Compensation for this role includes $80$90 per evaluation plus a mileage stipend and pay for administrative time.
