Job Description

Make a meaningful impact as an experienced Nurse Practitioner delivering high-quality home health risk assessments to Medicare Advantage members throughout Grant County. This per diem opportunity allows you to use your clinical expertise to support patient health in a flexible, independent environment.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Enjoy the autonomy of creating your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

  • Perform annual health risk assessments for Medicare Advantage plan members in their homes
  • Evaluate medical history, current conditions, and medication usage during every visit
  • Educate patients on health conditions and proper medication adherence
  • Identify gaps in care and provide recommendations, supporting patients ongoing wellness
  • Visits do not include diagnosis, prescribing, or developing treatment plans

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • MSN from an accredited nursing school
  • At least 6 months of recent nurse practitioner experience preferred

Compensation for this role includes $80$90 per evaluation plus a mileage stipend and pay for administrative time.
