Take on an impactful role providing home health risk assessments as a Nurse Practitioner within Northampton County, NC. This per diem opportunity offers a flexible schedule and the autonomy to arrange your own hours, making it ideal for those interested in supplementing their practice with meaningful, one-on-one care in home settings. Assignments may be in Gaston, Garysburg, or Rich Square, depending on shift needs.
Schedule:

Key Responsibilities:
- Perform comprehensive annual health and risk assessments for Medicare Advantage plan members
- Assess patients medical history, current conditions, and medication usage during home visits (average assessment: 45 minutes)
- Deliver clear education regarding health conditions and medication adherence
- Identify gaps in care and recommend actions to ensure holistic patient support
- No treatment plan creation or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
