Job Description
Deliver high-quality patient-centered care as a Nurse Practitioner providing home health risk assessments throughout Halifax County, NC. This per diem opportunity offers flexibility, autonomy, and meaningful impact as you partner in the health management of Medicare Advantage members during scheduled home visits across areas such as Roanoke Rapids, Littleton, Scotland Neck, Weldon, and more.
Schedule:
- Flexible shifts available Monday through Sunday
- Tailor your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual and post-discharge risk assessments for Medicare Advantage plan members in their homes
- Review and document patients prior and current medical history, chronic conditions, and medication use
- Educate patients on health conditions, wellness strategies, and their ongoing medication regimens
- Identify gaps in care and provide actionable recommendations for follow-up and care coordination
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification as NP (AANP or ANCC)
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- At least 6 months experience as an NP preferred
- Home health experience with a commitment to compassionate care
