Job Description
Make a significant difference in the lives of patients by providing comprehensive home health risk assessments as a Nurse Practitioner in a per diem capacity. This position offers a high degree of flexibility and autonomy, allowing you to create your own schedule for shifts between Monday and Sunday, with minimum weekly or bi-weekly hourly commitments. You will serve Medicare Advantage plan members throughout Halifax County, covering areas such as Roanoke Rapids, Littleton, Scotland Neck, Weldon, and the surrounding communities.
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate prior and current medical history, conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify care gaps and provide recommendations to support care continuity
- Focus exclusively on assessment; no treatment planning or prescribing
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC required
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- Minimum of 6 months current experience as an NP preferred
Enjoy compensation of $80-$90 per completed evaluation, as well as a mileage stipend and additional pay for administrative time.
