Step into a dynamic per diem opportunity tailored for Nurse Practitioners specializing in home health services. This role involves conducting comprehensive home health risk assessments for Medicare Advantage plan members across Lake County, IN, including locations such as Gary, Hammond, and Merrillville. Each assessment typically lasts around 45 minutes and may encompass annual wellness checks or post-discharge follow-ups. Providers will have flexibility in scheduling and will be assigned to various areas within North Township and the surrounding region during their shifts.

Schedule:

• Flexible shifts available Monday through Sunday
• Create a personalized work schedule
• Minimum commitment on a weekly or bi-weekly basis

Key Responsibilities:

• Perform annual risk assessments in patients homes for a Medicare Advantage plan
• Review and evaluate past/current medical conditions and medication histories
• Educate patients on their diagnoses, medications, and general health
• Identify care gaps and suggest appropriate follow-up actions or referrals
• No treatment planning or prescription responsibilities required

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current BLS (CPR) certification
• Board certified by AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months experience as an NP preferred
