Deliver vital care in the comfort of patients homes with a flexible per diem opportunity focused on home health risk assessment services. In this role, you will complete comprehensive visits for Medicare Advantage members in Lake County, IN, ensuring a positive impact on their health outcomes. Each home health risk assessment typically lasts about 45 minutes and may include annual health checkups or post-discharge visits. Your outreach will cover patients in Gary, Hammond, Merrillville, and nearby communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Control your own scheduling to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage members in their homes
- Review and document patients medical history, conditions, and medication use
- Educate patients about their diagnoses, medication, and preventive measures
- Identify care gaps; offer actionable recommendations to help address these needs
- Focus strictly on assessments and educationno prescriptive authority or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current BLS (CPR) certification
- Board-certified through AANP or ANCC
- Minimum six months of recent NP experience (preferred)
- MSN from an accredited nursing program
Youll be compensated at $80-$90 per evaluation, plus a mileage stipend and payment for administrative time.
