Offering a unique opportunity for a skilled Nurse Practitioner to deliver in-home health risk assessments, this per diem position provides flexibility and impactful patient interaction within Allen County, INincluding Fort Wayne, New Haven, and Huntertown. Engage directly with Medicare Advantage plan members, conducting comprehensive health evaluations that support proactive patient care and wellness.

Key Responsibilities:
- Perform thorough annual health risk assessments for Medicare Advantage members in a home setting
- Evaluate patient medical histories, current health status, chronic conditions, and medication regimens
- Identify gaps in care and offer meaningful recommendations
- Educate individuals on health conditions and medication management
- Visits may include annual assessments or supportive post-discharge evaluations
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Indiana Nurse Practitioner license
- Current BLS certification
- Board certification via AANP or ANCC required
- Minimum 6 months of NP experience preferred
- MSN degree from an accredited nursing program
