Job Description

Enjoy the freedom and variety of per diem work as you provide vital home health clinical assessments to Medicare Advantage members in Johnston County, NC. As a Nurse Practitioner specializing in home health risk assessments, you will conduct comprehensive evaluations in patient residences, helping to improve care outcomes and quality of life for individuals across Clayton, Smithfield, Selma, Cleveland, and the surrounding community.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-schedulingdesign your own workweek
  • Set your preferred minimum weekly or bi-weekly hours

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, medical conditions, and medication lists
  • Educate patients on their conditions and medications to foster informed care
  • Identify care gaps and offer actionable recommendations for follow-up
  • Conduct visits averaging 45 minutes each, with a focus on comprehensive assessmentno treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent nurse practitioner experience preferred
