Job Description

Deliver high-quality and compassionate care as a Nurse Practitioner focused on home health risk assessments with a per diem opportunity in Ector County, TX. This position is ideal for professionals with a commitment to flexible work and patient-centered service. Assessments will be conducted for Medicare Advantage plan members in their residences throughout Odessa, West Odessa, Goldsmith, and nearby areas, providing vital annual and post-discharge evaluations. Each assessment takes approximately 45 minutes and varies based on patient needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimal hourly commitment required each week or bi-weekly

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate individuals current and previous medical histories, chronic conditions, and medication regimens
• Deliver education to patients about their conditions and prescribed medications
• Identify care gaps and recommend solutions for improved health outcomes
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license valid in Texas
• Board Certification through AANP or ANCC
• Active CPR certification (BLS)
• MSN degree from an accredited nursing institution
• At least 6 months of recent nurse practitioner experience preferred
