Job Description

Make a meaningful impact in your community by providing home health risk assessments to Medicare Advantage plan members. This per diem opportunity offers exceptional flexibility and gives Nurse Practitioners the autonomy to create their own schedules while serving individuals across Cass County, including Logansport, Galveston, and Walton.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Freedom to design your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Complete annual home health risk assessments during home visits for Medicare Advantage members
  • Thoroughly evaluate prior and current medical history, conditions, and medication use
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care and make recommendations to improve patient outcomes
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of nurse practitioner experience is preferred
