Job Description

Experienced Nurse Practitioners are invited to apply for a per diem opportunity focused on conducting home health risk assessments throughout Cass County, Indiana, including areas such as Logansport, Galveston, and Walton. This flexible role is ideal for individuals who value autonomy and are passionate about providing high-quality care in a community-based setting. Each home health risk assessment typically lasts about 45 minutes and may involve annual checkups or post-discharge visits with Medicare Advantage plan members.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to create your own work schedule
  • Minimum commitment required weekly or bi-weekly

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate patients medical history, current conditions, and medication usage
  • Offer education and resources related to health conditions and medications
  • Identify and communicate gaps in care, recommending solutions as necessary
  • Focus solely on assessmentsno requirements for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience is preferred
  • Master of Science in Nursing (MSN) from an accredited institution
