Job Description

Experienced Nurse Practitioners are invited to provide comprehensive home health risk assessments for Medicare Advantage plan members in Meigs County, Ohio. In this per diem opportunity, you will conduct individual evaluationstypically lasting about 45 minutesin members homes throughout communities such as Middleport, Pomeroy, Syracuse, Racine, and the surrounding areas. The role offers flexibility to create your own schedule and serve a diverse patient population while supporting continuity of care.

Schedule:

  • Flexible shifts available any day of the week (MondaySunday)
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitments to fit your availability

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments
  • Evaluate current and prior medical history, health conditions, and medication usage
  • Deliver patient education regarding health conditions and medication adherence
  • Identify gaps in care and suggest follow-up actions to enhance patient outcomes
  • No responsibility for prescribing medication or developing treatment plans

Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current CPR certification (BLS required)
  • Board Certification as an NP through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited program
  • At least 6 months of current NP experience preferred

This per diem field-based role offers compensation of $80$90 per completed evaluation, plus a mileage stipend and additional compensation for administrative time.
