Deliver essential care and support in a rewarding per diem nurse practitioner role focused on home health risk assessments. This opportunity centers around conducting in-home evaluations for Medicare Advantage members across Richland County, Ohioincluding Mansfield, Ontario, and Madison Townshipwith flexibility to accommodate your preferred schedule.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete comprehensive annual health and risk assessments in patient homes
• Assess individual medical history, current diagnoses, and medications
• Educate patients about their conditions and prescribed medications
• Recognize gaps in care and communicate recommendations for follow-up
• Visits average 45 minutes each; no treatment plan creation or medication prescribing is required

Qualifications:

• Licensed Nurse Practitioner (NP) in Ohio
• Board Certification through AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of recent nurse practitioner experience preferred
