Advance your clinical career and support patient well-being by providing vital health assessments in a flexible per diem role. This position involves conducting home health risk assessments for Medicare Advantage plan members within Gainesville and the surrounding communities in Alachua County. This opportunity is ideal for Nurse Practitioners who excel in independent, patient-focused care and value a customizable work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members in their homes
- Evaluate and document prior and current medical history, medical conditions, and medication history
- Offer patient education on existing medical conditions and medications
- Identify gaps in care, and provide practical recommendations to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- Board certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of Nurse Practitioner experience is preferred
