Key Responsibilities:
- Perform comprehensive annual risk assessments during home visits for members of a Medicare Advantage health plan
- Review and document prior/current medical history and medication use
- Assess medical conditions, educate patients on diagnoses and medication regimens
- Identify potential gaps in care, recommending solutions when needed
- Focus exclusively on assessmentno prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification (AANP or ANCC)
- Current BLS/CPR certification
- MSN from an accredited school of nursing
- At least 6 months of recent NP experience preferred
- Experience in home health settings or risk assessments is a plus
This per diem Nurse Practitioner position offers competitive compensation of $80-$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
