Job Description
Make a meaningful difference in patient care by providing home health risk assessments as a Nurse Practitioner. This per diem opportunity offers a flexible schedule and the chance to work within Overton County, serving those in Livingston and neighboring communities.
Schedule/Hours:
- Flexible shifts offered Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members, including annual health assessments and post-discharge visits
- Review and evaluate each patients prior and current medical history, medical conditions, and medication usage
- Offer education on medical conditions and medications to enhance patient understanding and compliance
- Identify gaps in care, communicating clear recommendations for improvement
- No responsibilities for creating treatment plans or prescribing medicationsfocus solely on assessment and education
Required Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- 6+ months of recent nurse practitioner experience preferred
Compensation for this role ranges from $80 to $90 per completed evaluation, with a mileage stipend and administrative time also provided.
