Experienced Nurse Practitioners are sought for per diem, home-based health risk assessments in Yuma County, AZ. In this flexible position, you will perform comprehensive in-home health risk evaluations for Medicare Advantage members throughout the region. Each assessment lasts approximately 45 minutes and may involve annual wellness checks or follow-up visits post-discharge. Locations include Yuma, San Luis, Somerton, and other areas within Yuma County, with candidates scheduled in different communities across individual shifts.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members
• Review and document medical history, current conditions, and medication lists
• Educate members on chronic conditions and proper medication use
• Identify gaps in care and recommend appropriate interventions
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active AZ Nurse Practitioner License
• Board Certification via AANP or ANCC
• Current BLS/CPR certification
• Minimum 6 months of Nurse Practitioner experience preferred
• MSN degree from an accredited nursing school
