Job Description

Experienced Nurse Practitioners with a passion for home health are invited to conduct risk assessments for Medicare Advantage plan members. This per diem role offers flexibility as you provide essential health evaluations in a variety of home settings throughout Oswego County, including locations such as Fulton, Brewerton, and Pulaski.

Schedule:

  • Choose your own shiftswork as needed between Monday and Sunday
  • Build your schedule to fit your availability
  • Flexible minimum weekly or bi-weekly hour commitments

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage members
  • Assess medical, surgical, and medication history during 45-minute home visits
  • Educate patients on managing their health conditions and medication regimens
  • Identify gaps in care and make recommendations to address them
  • No treatment plans, diagnosing, or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN from an accredited nursing program
  • Minimum of 6 months current NP experience preferred

This position offers $80-$90 per completed evaluation, with an additional mileage stipend and compensation for administrative time.
