Experienced Nurse Practitioners with a passion for home health are invited to conduct risk assessments for Medicare Advantage plan members. This per diem role offers flexibility as you provide essential health evaluations in a variety of home settings throughout Oswego County, including locations such as Fulton, Brewerton, and Pulaski.
Schedule:
- Choose your own shiftswork as needed between Monday and Sunday
- Build your schedule to fit your availability
- Flexible minimum weekly or bi-weekly hour commitments
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Assess medical, surgical, and medication history during 45-minute home visits
- Educate patients on managing their health conditions and medication regimens
- Identify gaps in care and make recommendations to address them
- No treatment plans, diagnosing, or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing program
- Minimum of 6 months current NP experience preferred
This position offers $80-$90 per completed evaluation, with an additional mileage stipend and compensation for administrative time.
