Make a meaningful impact by providing personalized care in the comfort of patients homes. In this per diem opportunity, youll utilize your advanced clinical expertise to conduct home health risk assessments for Medicare Advantage plan members throughout Hempstead County, AR, including areas such as Hope, Blevins, and Perrytown.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Design your own work schedule
- Meet minimum hourly commitments on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual or post-discharge home health risk assessments
- Accurately assess and document patients prior and current medical histories, conditions, and medications
- Deliver patient-focused education on health status and medication usage
- Recognize gaps in patient care, offering actionable recommendations
- No requirement to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of current experience as a nurse practitioner (preferred)
Your work will help ensure high-quality healthcare delivery by identifying needs and closing care gaps for members in their own homes. In addition to rewarding clinical experiences and a supportive per diem structure, you will be compensated $80$90 per evaluation, including a mileage stipend and coverage for administrative time.
