This opportunity involves providing essential care to Medicare Advantage plan members throughout Payette County, including communities such as Payette, Fruitland, and New Plymouth. Schedule / Hours
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment, offering adaptable work-life balance
Key Responsibilities
- Complete annual home health risk assessments for Medicare Advantage health plan members during home visits
- Review and evaluate prior and current medical history, medical conditions, and medication use
- Educate patients regarding their conditions and prescribed medications during each assessment
- Recognize care gaps, and provide actionable recommendations for their resolution
- Focus solely on assessment and education; no treatment planning or prescription responsibilities
Qualifications
- Active Nurse Practitioner license in Idaho
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- At least six months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
