Schedule / Hours

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your availability
  • Minimum weekly or bi-weekly hourly commitment, offering adaptable work-life balance

Key Responsibilities

  • Complete annual home health risk assessments for Medicare Advantage health plan members during home visits
  • Review and evaluate prior and current medical history, medical conditions, and medication use
  • Educate patients regarding their conditions and prescribed medications during each assessment
  • Recognize care gaps, and provide actionable recommendations for their resolution
  • Focus solely on assessment and education; no treatment planning or prescription responsibilities

Qualifications

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • At least six months of NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program
