Schedule & Work Structure
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
- Assignments may occur in Grand Junction, Fruita, Palisade, or surrounding Mesa County areas
Key Responsibilities
- Perform thorough annual home health risk assessments for Medicare Advantage plan members
- Evaluate prior and current medical history, existing conditions, and medication usage
- Educate patients about their medical conditions, medications, and management strategies
- Identify care gaps and share actionable recommendations for effective follow-up
- No treatment plan development or prescription writing required
Qualifications
- Active Nurse Practitioner license in Colorado
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- MSN from an accredited school of nursing
- At least 6 months experience as a Nurse Practitioner preferred
