Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Choose a weekly or bi-weekly hourly commitment that suits your needs

Key Responsibilities:

• Perform thorough annual home health risk assessments for Medicare Advantage plan members in their homes
• Review prior medical history, conditions, and medications during each approximately 45-minute assessment
• Educate patients on their medical conditions and medication regimens
• Detect any gaps in care and recommend solutions to improve overall health outcomes
• Visits may include both annual health assessments and post-discharge check-ins
• No responsibilities in treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Colorado
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least six months of recent NP experience is preferred
