Job Description

Deliver personalized healthcare in the comfort of patients homes as a Nurse Practitioner specializing in home health services. This per diem opportunity involves performing risk assessments for Medicare Advantage members across Mesa County, including Fruita, Grand Junction, and Palisade. Enjoy control over your work-life balance by creating your own schedule, with flexible shifts available any day of the week.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Set your own working hours
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, averaging 45 minutes per visit
  • Assess and document past and current medical history, medical conditions, and medication regimens
  • Educate patients about their medical conditions and medications during risk assessment visits
  • Identify gaps in care and offer actionable recommendations to improve patient health outcomes
  • Visits may include annual health assessments or post-discharge check-ins
  • No requirements for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Board certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN from an accredited nursing program
  • Minimum of 6 months nurse practitioner experience preferred
