Make a meaningful impact by delivering expert care directly to patients homes in Warren County, VA and surrounding communities such as Front Royal and Bentonville. In this per diem Nurse Practitioner role, you will conduct home health risk assessments exclusively for Medicare Advantage plan members, helping to ensure patients receive the comprehensive care they deserve.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments (each approximately 45 minutes)
- Review and evaluate patients medical histories, current conditions, and medication regimens
- Provide thorough education on diagnoses and medication protocols
- Identify and document gaps in care, offering clear recommendations for follow-up
- Focus on assessment onlyno prescribing or treatment planning
Qualifications:
- Active Nurse Practitioner license in Virginia
- Current CPR/BLS certification
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- Minimum of 6 months current NP experience preferred
